Absence Form
Fill out this if you plan on missing a meeting. Please provide your name, reason for being absent, and date you will be absent. This form will be reviewed by our executive team to determine whether or not the absence is excused. Please remember you can not have more than three unexcused absences a semester or you will not be eligible for the retreat.听
Office Hours
