Vaccinations and boosters continue to be strongly recommended as a safe, effective tool for minimizing the effects of COVID-19, including serious illness and death.

To comply with 蜜桃传媒破解版下载 COVID-19 vaccine requirement, members of the campus community are required to have received the initial series of vaccine plus one booster, or file for an exemption.
Students, faculty and staff must upload proof of boosters or file for an exemption based on medical, ethical or moral grounds through the . Individuals who previously submitted an exemption do not need to take further action at this time and will be considered in compliance with current university vaccine requirements.
There are two ways to complete the vaccine requirement:
- Proof of vaccination: Only vaccines that are approved by the World Health Organization (WHO) will meet the CU vaccine requirement.
- Vaccine exemption: Personal exemptions will no longer be accepted. Only medical or moral/ethical exemptions are now allowed.
All faculty and staff must complete the COVID-19 vaccine requirement or exemption in the . The only exception will be faculty and staff vaccinated through 蜜桃传媒破解版下载 Medical Services. Data from Medical Services will be uploaded to the portal on behalf of faculty and staff vaccinated through Medical Services.
Providing proof of vaccination
Go to the , select 鈥淔aculty/Staff: Vaccine Compliance鈥 and login with your Identikey username and password.
Click the COVID-19 "Proof of COVID-19 Vaccination."
Select the vaccine you received from the dropdown menu and enter your vaccine date(s).
Upload an image of your COVID-19 Vaccine Card. Click 鈥淪ubmit鈥
Please note: Once you have submitted all required documentation, you will receive an automated confirmation email. If you have incomplete documentation or if additional information is needed, you will not receive a confirmation email and the immunizations team will contact you directly. The verification process may take 7-10 days.
How to complete an exemption
Faculty, staff and students who cannot or who choose not to get a vaccine can complete an exemption.
Note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from 蜜桃传媒破解版下载 and quarantine. The specifics may vary depending on the circumstances of the outbreak.
There are two types of exemptions:
- Medical exemption: Medical exemptions must be signed by a licensed physician and must include a date and a reason for exemption.
- Moral/Ethical exemption: A moral/ethical exemption can be requested based on moral or ethical beliefs as to what is right or wrong that are held sincerely with the strength of traditional religious views.

Go to employee portal, select "Faculty/Staff: Vaccine Compliance" and login with your Identikey username and password.
Step 2:
Select the medical or religious exemption you would like to complete:
Medical: Print and fill out the medical exemption form. Have your provider sign the document and submit a digital copy using the 鈥淯pload鈥 link in the MyCUHealth portal.
Moral/Ethical: Complete and sign the religious exemption form within the portal.
Step 3:
For medical exemption: Once your form is completed and signed by a physician, login to the . Click on the 鈥淯pload鈥 button at the top and follow the instructions to upload your form.
Please note: Once you have submitted an exemption, they are manually verified. The verification process may take 7-10 days. If you have incomplete documentation or if additional information is needed, the immunizations team will contact you directly. You will receive a secure message once the exemption has been verified.

Step 1:
